Here the word 'decision' is singular.
Please clarify with some more examples.
I have lot of confusion regarding these type of sentences. For example
1. My body temperature has raised
2. My body temperature have raised
I do not know which verb (has/have) I should use.

In general, if the noun is singular, it must agree with the verb. In this case, my body temperature = it = 3rd person singular.

Hello,
The verb that follows has/have must be in the participle (adjective) form. This is true in all cases (I think).
the correct sentence is:
2. My body temperature has risen

You should always check the words in a dictionary if you are not sure.
You seem to mix up
to raise/raised/raised
to rise/rose/risen
